If a Covered Employee acquires an Eligible Dependent through a qualifying event, that Eligible Dependent may be enrolled no later than 60 days after the date they became an Eligible Dependent.  The employee will need to complete the Health Benefits Enrollment Form.  Please visit the Benefits Division website at: http://www.vermontpersonnel.org/employee/benefits_health.php.  Or call 828-0648 for additional information.

 

Otherwise, the annual Open Enrollment period is during the month of November each year. Information about health plan options, costs and plan enhancements will be mailed to each employee’s home. If you would like to change health plans or add/remove dependents, you must complete and submit the Medical/Dental Enrollment Form that will arrive in your packet.
